An (updated) LGBTQ+ friendly survey template

A step by step template, and recommendations for usage

After the amazing feedback I received on my original post “How to make an LGBTQ+ friendly survey,” I realized that my critiques on a badly written questionnaire did not provide the full context of what an inclusive LGBTQ+ survey would look like. After further experience in diversity and data work, I would make some changes and recommendations to create a fully functional survey template that is LGBTQ+ friendly. These recommendations come from my own experience as a queer and transgender person in the LGBTQ+ community, as well as from sources like GLADD’s Media Reference Guide.

Similar to my first post, I provide some context and recommendations for asking questions about gender and sexuality. I have also included optional pop-up descriptions that can be used if a researcher is concerned about the familiarity their users have with the terms used. If you have any additional questions or revisions you would make, please leave them in the comments!

Sex

Questions about sex should only be asked in specific instances where genitalia or assigned sex at birth are relevant. For example, medical surveys or (in limited cases) dating applications. In all other instances where a researcher is looking for data…
